Get your Alcohol Server Permit
 
All volunteers, including chaperones, must be approved by the Mukilteo School District.

The Policy Receipt Form, Volunteer Applicant Disclosure Form and Washington State Patrol Form must be completed and turned in before you begin your service even if you have already completed them for another school.

Once you have been approved as a volunteer you need to “re-up” every two years by re-completing and turning in the Washington State Patrol Form and the Volunteer Applicant Disclosure Form.

Please write KPAB (and the name of any other group you volunteer for at Kamiak) on all forms.The forms are provided below.

Completed forms can be turned into the Kamiak High School Administration office, Program Liaison, the Chaperone Chair, or placed in the secure box in the Band Room.  They can also be mailed to:

The Washington State Liquor Control Board (WSLCB) requires that ALL Alcohol servers/managers/owners
Must have completed a State approved Alcohol Server’s Training class and
Possess a Class 12 or 13 Permit
