Chaperone Information
For those of you who are interested in being chaperones for any upcoming symphonic orchestra trips, including Spain, SKMEA, and/or Gresham, we need to be sure all of our paperwork etc. is up to date. There are 4 components involved. Three of them are one time only items, so if you have already done them once (WITH KAMIAK, other schools won't count, even within in the school district) you are all set. The fourth one has to be redone every other year (again, WITH KAMIAK, other schools won't count, even within in the school district).
Completed forms can be turned into the Orchestra Director, Orchestra Liaison or placed in the Band Box.

One time only requirements:
- In house chaperone training conducted by KPAB chaperone coordinators.
- Receipt Form for Policies 5025, 3445 and 3421, regarding Sexual Harassment, etc. These policies can be found in the chaperone section of the Kamiak Performing Arts Boosters website at http://kamiakarts.org/chaperone.html. Please read the each policy, on line, and then sign a hard copy of the form. Please return the form, as described above.
- The Volunteer Application Disclosure Form is also available on the KPAB website and is also available by hard copy.
Every other year requirement:
- The WSP form is also available on the Boosters website, http://kamiakarts.org/chaperone.html. Please note, you do not need to provide your SSN, you need only complete section C. If you completed one of these forms last year and returned it to Kamiak, you should be OK. If it was not completed in the last year, you may need to redo it.

Even if your schedule doesn't allow for you to chaperone right now, you may want to go ahead with these requirements, so that you will be ready to go when the opportunity presents itself in the future (unless your youngest child is a senior).
